You’ll find Hill-affiliated doctors in your neighborhood, near work, around exceptional community hospitals and at academic medical centers.

When you enroll in an HMO, Medicare Advantage or managed Medi-Cal plan, you will be asked to select a medical group and a Primary Care Physician (PCP). This is the doctor you will see for routine care. Through your Hill-affiliated doctor, you will have hassle-free referrals to nearby Hill specialists and have access to exceptional hospitals. Use our Physician Directory to choose a great primary care doctor. Inside each PCP's profile, you will find the information you need to complete the enrollment process.
Tips on Choosing a PCP
Primary Care Physicians include several different types of doctors. You may wish to refine your search to one of these areas, depending on your needs. Once you have selected a PCP, talk to him or her about your hospital preferences.
  • Family Practice provides primary care for all ages and conditions.
  • General Practice provides primary care for all ages and conditions, particularly those with multiple health issues.
  • Internal Medicine provides primary care, with a focus on adults, and those with multiple health issues.
  • Pediatric Primary care for infants, children and adolescents, usually up to 18 years of age.
  • Adolescent Medicine Primary care with an emphasis on caring for children, usually from the later years of elementary school through early college.

We’ve made it easier to request appointments, get prescriptions and view many lab results online.  Just register for free RelayHealth service and choose a participating physician, and you will be able to take care of many routine requests without picking up the phone. In addition, all Hill members can create a secure Personal Health Record for 24/7 access to medical information. This makes it easier to access and update vital health information from anywhere -- home, work or even on the road in an emergency.

Hill Physicians has agreements with more than three dozen hospitals across Northern California, including exceptional choices near you.  Talk to your primary care physician about your hospital preferences. Your doctor will have privileges at some hospitals and, depending on your needs, may also refer you to specialists who use other hospitals. By discussing your preferences ahead of time, you and your doctor can choose a high-quality hospital that is accessible through your insurance plan and well-suited for your needs. In an emergency, call 911 or go to any nearby emergency room.

We know that minor illnesses and injuries can occur on weekends and at night. For those times when your primary care physician isn't available, Hill members can use an Urgent Care Center. These centers only provide non-emergency care, but they can save time and money compared to going to a crowded emergency room for non-emergency care.

Already Have an HMO?
If you already have an HMO plan, you can switch your medical group designation to Hill Physicians with just one phone call. There’s no change in your cost or interruption in your insurance coverage.

Enrolling in an HMO?
When you enroll in an HMO, you will need to indicate Hill Physicians as your medical group in order to receive the advantages of Hill membership. Your employer’s benefit packet may use a paper enrollment form or an online enrollment process.
If You Enroll Using A Paper Form, simply write in “Hill Physicians” for medical group/IPA. Then use our Physician Directory to choose a primary care physician near you. Inside each doctor's listing, you will find the Enrollment ID you need.
If You Enroll Online, use our physician directory to look for a new doctor or one that you already know. Within each physician’s listing you will find easy instructions on how to complete your enrollment process.  You can even email the information to yourself for easy reference.
We Will Help You Find A Doctor’s HMO ID. Health plans assign unique numbers to every medical group and every physician. These numbers indicate a medical group preference and your choice of a primary care doctor. You will need to use these numbers when enrolling with your HMO.
Our physician directory provides these numbers. When you search for a Primary Care Physician, just look inside the doctor's listing for enrollment information.
